ONLINE S2 APPLICATION

 

WHO MAY AVAIL OF THE S2 LICENSE ONLINE?

Renewal S2 applicants, i.e. Physicians, Dentists and Veterinarians who have applied personally at PDEA and whose information were already entered into the automated system.

 

WHAT ARE THE REQUIREMENTS?

1.     PRC ID Card or duly Certified True Copy

2.     PTR Official Receipt / latest Certificate of Employment, if government, physician, or duly Certified True Copy

3.     TIN card or latest ITR or duly Certified True Copy

4.     Drug Test Result (DOH-DDB IDTOMIS-generated report)

5.     Two pieces 1” x 1” ID picture (however for online application, upload picture.  If unable, submit 2” x 2” ID picture)

6.     Previous S2 ID issued, if applicable.  For lost ID, submit proof of publication of loss

Notes:

a)       Personal appearance is required to facilitate entry and capture of current photo and information into the automated system.  Once entered, need not apply in person on succeeding period.

b)      Government physicians are exempted from payment of license fee, however, must submit original duly-notarized affidavit that said S2 license shall be used exclusively in government practice only.

Click ‘S2 Application’

Click ‘Apply Now’ button

Log-in to the Systems, i.e. enter Username and Password.  If unable to recall, contact CS or text PDEALOGIN <S2#> to 0917-8277332 (0917-8CSPDEA)

Fill out the Application Online Form completely

Upload picture

Click ‘Print’ button after completing all the required fields

Log-out of the system

Got to nearby courier service company and send the (a) printed form, (b) required documents and (c) fees, in cash – please refer to printed form.  The fee depends on PRC validity.  PVC ID fee is optional—(d) pre-paid self-addressed return envelope to the PDEA Compliance Service, 2nd Floor PDEA Building, NIA Northside Road, National Government Center, Barangay Pinyahan, Quezon City.

 

Other forms of payment (thru Landbank, etc.) will be made available soon

 

 

 

 

 

Receive document envelope from courier.  Enter information in logbook and forward document to DDRO on duly

Check contents of envelope for completeness.  Call applicant for any discrepancy.  If ok, retrieve subject application # in database and process application.  Pay at the cashier
